Family Access Help
With family access, you can view another family member’s Guthrie medical record and communicate on their behalf.
Submit Online: Parents or guardians can send a picture or completed the form and send to CentralScheduleFax@mblayst.com as a faster way to have proxy completed.
To request parent access for a child under the age of 13, you must:
- Be the birth parent or legal guardian of the child
- Have access to your own eGuthrie account
- Print, read and complete the authorization form
A child age 13 to 17 may view their own medical records by signing up for their own eGuthrie account. Parental permission is required on the consent form.
For children ages 13 to 17, parents have limited access to information regarding allergies and immunizations only. For full parent access, the child must sign and submit an authorization form.
Parents automatically lose access when a child turns 18.
To request access for another adult, you must:
- Provider Power of Attorney documentation or obtain patient authorization
- Have access to your own eGuthrie account
- Print, read and complete the appropriate authorization form
